SMS Consent & Messaging Policy

Consent to Receive Text Messages
By providing your mobile phone number to Synergy Health DPC and opting in during patient enrollment, appointment scheduling, or through our patient portal, you expressly consent to receive text messages (SMS and MMS) from Synergy Health DPC related to your healthcare. These messages may include:
- Appointment reminders, confirmations, and scheduling updates
- Prescription ready notifications
- Lab result availability alerts
- Billing and payment reminders
- Care coordination and follow-up messages
- General practice announcements and health information
You are not required to consent to text messaging as a condition of purchasing any goods or services. Consent can be revoked at any time.
How to Opt In
You may opt in to receive text messages from Synergy Health DPC through any of the following methods:
- Patient enrollment: By providing your mobile phone number and checking the SMS consent box during new patient registration at synergyhealthdpc.com/enroll.
- Patient portal: By enabling text message notifications in your communication preferences within the SynergyEMR patient portal.
- In-office: By providing written consent on our intake forms during your visit.
- Verbal consent: By confirming your consent verbally during a phone call or telehealth visit with our staff.
How to Opt Out
You may opt out of receiving text messages at any time using any of these methods:
- Reply STOP to any text message you receive from us. You will receive a one-time confirmation that you have been unsubscribed.
- Patient portal: Disable text message notifications in your communication preferences.
- Contact us: Call (224) 341-5202 or email info@synergyhealthdpc.com to request removal from our text messaging list.
Opting out of text messages will not affect your ability to receive healthcare services, access your medical records, or use the patient portal.

Message Frequency
Message frequency varies based on your appointments, prescriptions, and healthcare activities. You may receive up to 15 messages per month depending on your care needs. Recurring messages will be sent for appointment reminders, prescription alerts, and other healthcare communications as described above.
Message & Data Rates
Message and data rates may apply. Your mobile carrier’s standard messaging rates apply to all text messages sent to and from our service. Synergy Health DPC is not responsible for any charges incurred from your mobile carrier.
Supported Carriers
Our text messaging service is compatible with all major U.S. carriers, including but not limited to AT&T, Verizon, T-Mobile, Sprint, U.S. Cellular, Cricket, MetroPCS, Boost Mobile, and other regional carriers. Carrier support may vary and carriers are not liable for delayed or undelivered messages.
Privacy & Security
Your phone number and messaging data are handled in accordance with our Privacy Policy. We do not sell, share, or rent your phone number or opt-in data to third parties for marketing or promotional purposes. Message data, including phone numbers and consent records, are retained as required by law and in accordance with HIPAA regulations.
SMS/MMS messages are not encrypted in transit and may be accessible by your mobile carrier. We will not include detailed Protected Health Information in text messages. Messages will contain general reminders and alerts, directing you to your secure patient portal for detailed clinical information.
